Department of Radiology, Medical College of Ohio, Toledo, Ohio Endometriosis of the abdominal wall usually occurs in a scar, and rarely in the intact rectus abdominis muscle. A 38-year-old woman presenting with an abdominal wall mass, which was cyclically tender with her menstrual periods, had an endometrioma not related to a previous scar. Based on a sonogram, the mass was solid, hypoechoic, and situated entirely within the rectus muscle. The differential diagnosis includes abscess, hematoma, and neoplasm, but the cyclical tenderness and enlargement of the mass confirms the diagnosis.
